An opportunity has arisen for a full time Peripatetic Team Manager, within Central Urgent Care, Acute and Prisons service managing our peripatetic hospital discharge team. The post you are applying for is a full-time permanent post based at county hall with a future vision to align the post to Fylde and Wyre.

The Acute discharge Team utilises the Discharge to Assess pathway and champions the Home First ethos. The team strives to maintain patient flow and to provide better lives for the people of Lancashire. Supporting more people to return home which will include complex discharges. Reducing delays/long length of stay.
The successful candidate will become an integral part of a countywide urgent care, acute and prisons management team, working in partnership with Lancashire County Council and the NHS. We are a fluid and integrated resource that assists in relieving hospital pressures. We work in partnership with our health colleagues, Age UK, Housing colleagues and carers services.
Your role will include:
Manage a busy front-line service.
Respond to referrals urgent in nature and directing staffing resource.
Manage demand, manage risk and competing priorities.
Be able to challenge and negotiate.
Offer direct line management support to the senior social worker, social workers / SCSO's, including formal supervision, HR oversight, case work allocation.
Manage complaints.
Case work oversight, budget approvals/ oversight.
Contribute to the management of performance and data; the demands across the area- reporting to Area Manager
Lead the team Case Progression and Wellbeing Team Meeting
Be a supportive team member to wider Acute management team.
Attend and contribute to the area meetings led by the County Operations Manager.
Liaise and work jointly with other colleagues, internal and external such as health and contribute to the Integration agenda.
Ensure a culture of promoting independence is embedded within the team.
Undertake all other relevant management tasks as directed
